Comparison of chest radiographs with 2K vs 4K high-resolution soft-copy images: an ROC study

Abstract
OBJECTIVE: Computed radiography of chest with a 4K image array was recently introduced. We performed a multiobserver study to compare the diagnostic accuracy of 2K (standard) and 4K (HQ) chest radiographs displayed on 5 Mega pixel monitors (2K monitors).

SUBJECTS AND METHODS: Fifty cases of posteroanterior chest radiographs (a total 100 images) selected by two chest radiologists to contain pnuemothorax (n=14), nodule (n=15), interstitial disease (n=10) and neither abnormality (n=11) were interpreted by four radiologists in two separate sessions. They recorded their confidence scale for the presence or absence of above abnormalities. Diagnostic accuracy was determined by receiver operating characteristic (ROC) analysis for each observer.

RESULTS: ROC analysis showed no statistically significant difference between 2K and 4K modes of chest for the detection of any of the different abnormalities by each observer.

CONCLUSION: Our preliminary study suggests that there is no considerable propriety to incline to the 4K mode of chest in present PACS environment using 2K video monitor. But we think that additional investigation in cases of very subtle parenchymal abnormality or rib lesion on the chest radiograph should be followed.
